Distribution of Amount of Medical Care in the Compulsory Health Insurance (CHI) System: the Case of St. Petersburg

Abstract

Исследование посвящено анализу распределения объемов медицинской помощи в системе обязательного медицинского страхования на примере города Санкт-Петербург. В настоящее время в России существуют механизмы выделения объемов медицинской помощи медицинским организациям. Сейчас это распределение в большинстве своем характеризуется «пассивным подходом». Он заключается в том, что объемы медицинской помощи распределяются в определенных размерах, опираясь на исторические данные: размеры, сроки работы, коечный фонд, укомплектованность штата медицинских работников медицинской организации и др., а не на эффективность их деятельности. В тексте работы предложена новая система распределения, характеризующаяся «стратегическим подходом», которая подразумевает фокус на показатели результатов работы медицинской организации и создание для них позитивных экономических стимулов. Идеи, на которые опирается «стратегический подход» распределения объемов медицинской помощи в системе ОМС, коррелируют с концепцией Нового государственного менеджмента — New Public Management. В работе исследуется следующая гипотеза: в Санкт-Петербурге используется «пассивный подход» к распределению объемов медицинской помощи в системе ОМС для реализации большинства медицинских процедур. Целью данного исследования является разработка комплекса мер по расширению применения «стратегического подхода» к распределению объемов медицинской помощи в системе ОМС в городе Санкт-Петербург. В ходе работы были проведены глубинные интервью с экспертами по ОМС, в результате которых была получена ценная информация. Результаты данной работы могут быть использованы государственными органами управления здравоохранением и Комиссиями по разработке территориальной программы обязательного медицинского страхования субъектов РФ для повышения эффективности и результативности распределения объемов медицинской помощи в системе ОМС на примере города Санкт-Петербурга, а также медицинскими организациями для предоставления корректной информации об их деятельности по запросу органов управления здравоохранением.

The research is devoted to the analysis of the distribution of health care amounts in the compulsory health insurance system using the example of the city of St. Petersburg. Currently, in Russia, there are mechanisms for allocating amounts of health care to medical organizations. Now, this distribution is mostly characterized by a “passive approach”. It lies in the fact that the amounts of health care are distributed in certain sizes, based on historical data: size, terms of work, bed capacity, staffing of medical workers of a medical organization, etc., and not on the effectiveness of their activities. The text of the work proposes a new distribution system, characterized by a “strategic approach,” which implies a focus on the performance indicators of a medical organization and the creation of positive economic incentives for them. The ideas on which the “strategic approach” of distributing the amounts of health care in the compulsory health insurance system is based correlate with the concept of New Public Management. The paper examines the following hypothesis: in St. Petersburg, a “passive approach” is used to distribute the amounts of health care in the compulsory health insurance system for the implementation of most medical procedures. The purpose of this study is to develop a set of measures to expand the use of a “strategic approach” to the distribution of amounts of health care in the compulsory health insurance system in the city of St. Petersburg. During the work, in-depth interviews were conducted with compulsory health insurance experts, as a result of which valuable information was obtained. The results of this work can be used by state health authorities and Commissions for the development of a territorial program of compulsory health insurance for constituent entities of the Russian Federation to improve the efficiency and effectiveness of the distribution of amounts of health care in the compulsory health insurance system using the example of the city of St. Petersburg, as well as by medical organizations to provide correct information about their activities at the request of health authorities.
