Organisation of medical care and quality of life evaluation in patients with diabetes mellitus in the Russian Federation

Abstract

Дипломная работа посвящена проблемам оценки качества жизни больных сахарным диабетом в различных субъектах Российской Федерации. Актуальность выбранной темы не вызывает сомнения, так как в Российской Федерации сахарный диабет является одним из самых распространенных среди всех заболеваний. Исследование представляет собой анализ данных анкет, предназначенных для оценки медико-социального статуса, образа жизни, качества организации медицинской помощи лиц, страдающих сахарным диабетом в различных субъектах Российской Федерации. Проведен ретроспективный анализ данных анкет 76 условно здоровых людей, 135 пациентов с сахарным диабетом 1 и 2 типов. Опрос проводился в Интернете. В первой части нашей работы с помощью статистического анализа обработаны ответы населения об осведомленности о заболевании сахарный диабет, были выяснены возможные факторы риска сахарного диабета, предрасположенности к сахарному диабету, первоначальные симптомы у здоровых пациентов и отсутствие настороженности со стороны врачей первого контакта. Во второй части исследования мы проанализировали уровень, доступность и качество медицинской помощи, наличие кадрового потенциала. Оценено медико-социальное благополучие пациентов с сахарным диабетом, их образ жизни, наличие диабетических школ для пациентов. Выявлена нехватка кадров в поликлиниках, прежде всего эндокринологов, отсутствие школ диабета в регионах, недостаточное обеспечение пациентов бесплатными средствами самоконтроля и расходными материалами к ним, что является основными проблемами в организации медицинской помощи лицам с сахарным диабетом. Данное исследование позволило точнее оценить потребности граждан, болеющих сахарным диабетом, пути снижения возможных осложнений этого серьезного заболевания, влияющего на трудоспособность, инвалидность и общую смертность в РФ.

The thesis is devoted to the problems of assessing the quality of life of patients with diabetes mellitus in various subjects of the Russian Federation. The relevance of the chosen topic is beyond doubt, since diabetes mellitus is one of the most common among all diseases in the Russian Federation. The study analyze the data of questionnaires designed to assess the medical and social status, lifestyle, quality of medical care organization for people suffering from diabetes mellitus in various subjects of the Russian Federation. A retrospective analysis of the questionnaire data of 76 contingently healthy people, 135 patients with type 1 and type 2 diabetes mellitus was carried out. The survey was conducted on the Internet. In the first part of our work, with the help of statistical analysis, the responses of the population about awareness of the disease diabetes mellitus were processed, possible risk factors for diabetes mellitus, predisposition to diabetes mellitus, initial symptoms in healthy patients and the lack of alertness of first-contact doctors were clarified. In the second part of the study, we analyzed the level, availability and quality of medical care, the availability of human resources. The medical and social well-being of patients with diabetes mellitus, their lifestyle, the presence of diabetic schools for patients were evaluated. The shortage of staff in polyclinics, primarily endocrinologists, the absence of diabetes schools in the regions, insufficient provision of patients with free self-monitoring tools and consumables for them, which are the main problems in the organization of medical care for people with diabetes mellitus, was revealed. This study made it possible to more accurately assess the needs of citizens suffering from diabetes mellitus, ways to reduce possible complications of this serious disease affecting working capacity, disability and overall mortality in the Russian Federation.
