Infectious complications after tooth extraction and dental implant surgery

Abstract

Все более широкое внедрение методов дентальной имплантологии, появление новой специализации хирургов – стоматологов, такой, как хирург-стоматолог-имплантолог, поставило перед стоматологическим сообществом важнейшую практическую проблему – качественной предоперационной подготовки пациентов с целью обеспечения достаточных условий для внедрения имплантата в беззубый участок зубного ряда, а также проведения комплекса мер, направленных на профилактику развития возможных ранних и поздних гнойно-воспалительных осложнений как на этапе удаления зуба, так и на последующем этапе возмещения дефекта зубного ряда. Цель исследования На основании клинических данных изучить причины и распространенность инфекционных осложнений операции удаления зуба и дентальной имплантации у лиц, обратившихся за стоматологической помощью в стоматологическое отделение ММЦ «Согаз» за 2016 год. Задачи исследования 1) Изучение общих и местных послеоперационных осложнений операции удаления зуба на основании данных, полученных в ходе анализа современной медицинской литературы. 2) Изучение ранних и поздних послеоперационных осложнений операции дентальной имплантации на основании данных, полученных в ходе анализа современной медицинской литературы. 3) На основании клинических данных определить частоту, структуру и причины возникновения инфекционных осложнений после операции удаления зуба. 4) На основании клинических данных определить частоту, структуру и причины возникновения инфекционных осложнений операции дентальной имплантации. Материалы исследования Для решения поставленных задач на базе отделения стоматологии частной клиники ММЦ «Согаз» были обследованы 450 пациентов, обратившихся за 2016 год. Было выбрано 276 пациентов. В исследовании приняли участие пациенты в возрасте от 18 до 73 лет, 152 мужчин и 124 женщины. Основными критериями для отбора пациентов были: наличие включенных или концевых дефектов зубного ряда, с показаниями к плановому удалению зубов или с составленным планом лечения с целью дальнейшей дентальной имплантации, и значением уровня гигиены полости рта по индексу Грина-Вермиллиона не менее 1 балла. Выводы 1) На основании данных полученных в ходе анализа медицинской литературы посвященной инфекционным осложнениям после операции удаления зуба, выявлено что отсутствие адекватного лечения любых инфекционных осложнений операции удаления зуба может иметь сильное негативное воздействие на организм пациента, имеющее характер усугубляться до летального исхода. 2) Исходя из данных полученных в ходе анализа медицинской литературы посвященной инфекционных осложнениям операции дентальной имплантации, можно судить о том, что «не приживаемость» имплантата в тканях пациента может зависеть не только от невыполнения пациентом послеоперационных назначений, но и от не соблюдения протоколов операций и не полного предоперационного обследования хирургом. 3) На основании клинических данных в качестве инфекционных осложнений операции удаления зуба проведенной в амбулаторных стоматологических условиях был выявлен только альвеолит, с частотой распространенности 5% от общего количества пациентов. Основной причиной развития данного заболевания стало не соблюдение пациентами методов послеоперационного ведения лунки удаленного зуба. 4) На основании клинических данных в качестве инфекционных осложнений операции дентальной имплантации, проведенной в амбулаторных стоматологических условиях были дезинтегрированы 3 имплантата, 0,69 % от общего количества установленных дентальных имплантатов. Основной причиной развития данных осложнений стали не выполнения пациентами послеоперационных назначений и не полное предоперационное обследование, и клиническая подготовка пациента. Выпускная квалификационная работа выполнена на 79 страницах, включает в себя введение, основную часть, состоящую из трех глав, заключение, выводы по всем поставленным задачам и список литературы, состоящий из 54 литературных источников; содержит 6 таблиц и 8 рисунков.

Broad introduction of dental implantology techniques, appearance of a new specialization in dental surgery called dental surgeon-implantologist, posed the most important practical problem for the dental community. This is the problem of the qualified preoperative preparation of the patients in order to provide sufficient conditions for the implant insertion into the toothless area of the dentition, as well as the implementation of a set of measures aimed at prevention of a possible early and late purulent-inflammatory complications development, both at the stage of a tooth extraction, and at the subsequent stage of a defect in the dentition compensation. Purpose of the study Examine the causes and prevalence of the infectious complications during the tooth extraction operation and dental implantation among people applied for dental care to the dental office of the IMC "Sogaz" during 2016. Objectives of the study 1) To study general and local postoperative complications of the tooth extraction operation on the basis of the data obtained during the analysis of the modern medical literature. 2) To study early and late postoperative complications of the dental implant surgery on the basis of the data obtained during the analysis of the modern medical literature. 3) Determine the frequency, structure and causes of infectious complications of a tooth extraction operation on the basis of a clinical data. 4) Determine the frequency, structure and causes of infectious complications of a dental implantation surgery on the basis of a clinical data. Study materials In order to solve the set tasks on the basis of the department of dentistry of the IMC "Sogaz", 450 patients who applied for the year 2016 were examined. 276 patients were selected. The study involved patients aged from 18 to 73 years, 152 men and 124 women. The main criteria for the patients’ selection were: the presence of inclusions or end defects of the dentition, with indications for the teeth removal or with the prepared treatment plan for the further dental implantation, and the value of the oral hygiene level at the Green-Vermillion index of at least 1 point. Conclusions 1) On the basis of the data obtained during the analysis of the medical literature about infectious complications after the tooth extraction operation, it was found that the lack of adequate treatment of any infectious complications of the tooth extraction operation can have a strong negative impact on the patient's health, which could be aggravated up to a lethal outcome. 2) Proceeding from the data obtained during the analysis of the medical literature about the infectious complications of the dental implantation, it can be concluded that the "immovability" of the implant in the patient's tissues can be the result not only on the patient's violation of postoperative appointments, but also on the failure to comply with the operation protocols and incomplete preoperative examination by a surgeon. 3) On the basis of the clinical data, only alveolitis was detected as an infectious complication of a tooth extraction performed in outpatient dental conditions, with a prevalence rate of 5% from the total number of patients. The main reason of this disease development was patients’ non-compliance with the methods of postoperative care of removed tooth socket. 4) On the basis of the clinical data, 3 implants or 0.69% of the total number of established dental implants, were disintegrated as the infectious complication of the dental implantation operation performed in the outpatient dental conditions. The main reason for the development of these complications was patients' non-fulfillment of postoperative prescriptions and incomplete preoperative examination, and clinical preparation of the patient. The thesis is completed on the 79 pages and it includes an introduction, the main part consisting of three chapters, conclusions on all objectives assigned and a list of literature consisting of 54 literature sources. Also there are 6 tables and 8 figures.
