dc.description.abstract | Цель исследования: установить особенности течения и лечения спонтанных гематом в период COVID-19. Материалы и методы: Проанализированы результаты лечения 55 больных COVID-19 со спонтанными гематомами в 2 учреждениях - ФГБУ «Северо-Западный окружной научно-клинический центр имени Л.Г. Соколова Федерального медико-биологического агентства» (СЗОНКЦ) и СПб ГБУЗ «Городская Мариинская больница». Средний возраст составил 69,4±5,6 лет. Все пациенты поступили в тяжелом и средне-тяжелом состоянии и с 1-го дня госпитализации получали низкомолекулярные гепарины. У 21 (38%) пациентов объем поражения легких соответствовал КТ-4. Объем поражения, соответствующий КТ-3, наблюдался у 9 (18%) больных. У 23 (40%) объем поражения легких классифицирован как КТ-2. КТ-1 у 2 (4%) пациента. Методами диагностической визуализации гематом были МСКТ, УЗИ, ангиография и лапароскопия. Результаты: Гематомы у больных COVID-19 были представлены глубокими межмышечными гематомами. Характерной локализацией были гематомы влагалища прямой мышцы живота (29 больных). Реже выявлены гематомы грудной стенки (9 больных) и бедра (5 пациентов), голени (1 больной) и плеча (1 пациент). Забрюшинная гематома наблюдалась у 7 пациентов, гематома ягодичной области – у 3. Гемотрансфузия проведена у 48% больных. Факторами риска образования гематом были: исходно критический объем или прогрессирование объема поражения легких у пациентов с COVID-19, длительная антикоагулянтная терапия, коморбидность пациентов, наличие ожирения. Из 55 пациентов прооперировано 14 (26%) человек, эмболизация проведена 9 (16%)пациентам, пункции гематом выполнены у 6 (11%) больных и у 26 (47%) пациентов проводилась консервативная терапия с выполнением УЗИ-мониторинга состояния гематомы. Летальность составила 43,6% (24 больных). Заключение: У пациентов с COVID-19 могут развиваться гематомы различных локализаций. Тактика ведения больных должна определяться тяжестью состояния пациента по основному заболеванию – COVID-19, локализацией гематомы, величиной кровопотери, динамикой объема гематомы и выраженностью жидкостного компонента в ней по данным УЗИ или КТ. Развитие гематом при COVID-19 ассоциировано с плохим прогнозом. | ru_RU |
dc.description.abstract | The purpose of the study: to establish the features of the course and treatment of spontaneous hematomas during COVID-19. Materials and methods: The results of treatment of 55 patients with COVID-19 with spontaneous hematomas in 2 institutions were analyzed - The Federal State Budgetary Institution “North-Western district scientific and clinical center named after L. G. Sokolov Federal Medical and Biological Agency” and Saint-Petersburg State Budgetary Healthcare Institution “City Mariinsky Hospital”. The mean age was 69.4±5.6 years. All patients were admitted in severe and moderate condition and received low molecular weight heparins from the 1st day of hospitalization. In 21 (38%) patients, the volume of lung lesions corresponded to CT-4. A lesion volume corresponding to CT-3 was observed in 9 (18%) patients. In 23 (40%), the volume of lung involvement was classified as CT-2. CT-1 in 2 (4%) patients. The methods of diagnostic visualization of hematomas were MSCT, ultrasound, angiography and laparoscopy. Results: Hematomas in patients with COVID-19 were represented by deep intermuscular hematomas. Typical localization was hematoma of the vagina of the rectus abdominis muscle (29 patients). Hematomas of the chest wall (9 patients) and thigh (5 patients), lower leg (1 patient) and shoulder (1 patient) were less common. Retroperitoneal hematoma was observed in 7 patients, hematoma of the gluteal region - in 3 patients. Hemotransfusion was performed in 48% of patients. Risk factors for hematoma formation were: initial critical volume or progression of lung damage in patients with COVID-19, long-term anticoagulant therapy, patient comorbidity, and obesity. Out of 55 patients, 14 (26%) patients were operated on, embolization was performed in 9 (16%) patients, hematoma punctures were performed in 6 (11%) patients, and conservative therapy was performed in 26 (47%) patients with ultrasound monitoring of the hematoma. Mortality was 43.6% (24 patients). Conclusion: Patients with COVID-19 may develop hematomas of various locations. The tactics of managing patients should be determined by the severity of the patient's condition for the underlying disease - COVID-19, the location of the hematoma, the amount of blood loss, the dynamics of the volume of the hematoma and the severity of the fluid component in it according to ultrasound or CT. The development of hematomas in COVID-19 is associated with a poor prognosis. | en_GB |
