The impact of working conditions on the morbidity rate

Abstract

Введение: Проблема заболеваемости работников добывающей и обрабатывающей промышленности не теряет своей актуальности.​ Несмотря на автоматизацию трудового процесса, сохраняется воздействие вредных производственных факторов на представителей рабочих специальностей, подавляющее большинство из которых - мужчины. В отечественной и зарубежной научной литературе последних лет достаточно широко представлена информация о заболеваемости трудоспособного населения в целом, и о распространенности профессиональной патологии у трудящихся в сфере машиностроения.Кроме того, большое внимание в литературе уделяется вкладу экологических факторов в изменение показателей здоровья населения. Так, крупные промышленные предприятия добывающего и обрабатывающего комплексов являются основным источником загрязнения в регионах с неблагоприятной экологической обстановкой. Однако, как правило, не учитывается тот факт, что значительная часть инвалидов и лиц, не доживающих до пенсионного возраста, – это бывшие работники этих предприятий, иными словами, условия труда не всегда рассматриваются как фактор риска. Сведений о заболеваемости работников отрасли машиностроения в Санкт-Петербурге не опубликовано. В связи с этим представляется интересным исследовать эти показатели и сравнить с заболеваемостью по популяции в целом. Цель: Изучить влияние условий труда на уровень заболеваемости среди лиц трудоспособного возраста на примере работников, занятых во вредных условиях труда автомобилестроительного завода в Санкт-Петербурге. Методы: В рамках данной работы было проведено ретроспективное исследование трех групп сравнения: группа 1 (n = 1999 мужчин в возрасте 20-60 лет, медиана возраста - 32 года) - работники автомобилестроительного предприятия в Санкт-Петербурге, прошедшие в 2019 году периодический медицинский осмотр в ООО «МЦ Эко – безопасность»; группа 2 (n = 424 770 мужчин в возрасте 21-60 лет, медиана - 38 лет) - лица трудоспособного возраста, которые в 2019 году прошли диспансеризацию в медицинских организациях Санкт-Петербурга по месту прикрепления; группа 3 (n = 604 668 мужчин в возрасте 21-99 лет, медиана возраста 44 года) - взрослое население Санкт-Петербурга, прошедшее в 2019 году диспансеризацию аналогично группе 2. Межгрупповой анализ данных осуществлялся с помощью программного обеспечения MS Excel 2019. Статистическая значимость различий оценивалась с помощью критерия хи-квадрат Пирсона. Различия считались статистически значимыми при значении p <0,05. Результаты:В ходе исследования была изучена заболеваемость взрослого населения Санкт-Петербурга, в структуре которой в порядке убывания преобладают следующие рубрики: «Болезни системы кровообращения» (50,1%), «Болезни органов эндокринной системы» (14,6%), «Болезни органов пищеварения» (6,7%) и «Болезни органов дыхания» (4,0%). Структура заболеваемости у лиц трудоспособного возраста по преобладающим заболеваниям идентична заболеваемости взрослого населения Санкт-Петербурга. Основные отличия заключаются в процентных соотношениях между группами заболеваний: «Болезни системы кровообращения» (41,4%), «Болезни органов эндокринной системы» (16,9%), «Болезни органов пищеварения» (9,8%) и «Болезни органов дыхания» (4,7%). Тем не менее, в сравнении с группой трудоспособного населения обращает на себя внимание тенденция более высокой заболеваемости взрослого населения Санкт-Петербурга по всем классам, кроме инфекционных болезней и заболеваний нервной системы. Достоверно выше заболеваемость взрослого населения Санкт-Петербурга болезнями системы кровообращения. Заболеваемость работников автомобилестроительного завода значительно отличается от уровней заболеваемости лиц трудоспособного возраста в Санкт-Петербурге как по структуре, так и в абсолютных значениях. В структуре заболеваемости этого контингента преобладают болезни глаза и его придаточного аппарата, занимая 22,5%; на болезни костно-мышечной системы и соединительной ткани приходится 21,2%; далее идут патологии из рубрик «Болезни органов дыхания» (14,7%) и «Болезни системы кровообращения» (14,4%). При сравнении по абсолютным величинам у работников предприятия достоверно более высокие, чем у трудоспособного населения, уровни выявления заболеваний по всем рассмотренным в данной работе классам МКБ 10, кроме болезней нервной и мочеполовой систем, заболеваемость по которым достоверно ниже. Причинами такого различия могут быть вредные и/или опасные условия труда на автомобилестроительном заводе, особенности образа жизни работников или же различия в проведении профилактических медицинских осмотров в сравнении с диспансеризацией. Заключение: Здоровью работников, занятых на работах с вредными и/или опасными условиями труда следует уделить особое внимание. Огромное влияние на сохранение здоровья работающих оказывает внедрение корпоративных программ укрепления здоровья, что является мощнейшим механизмом превентивных мер в отношении развития как профессиональных, так и других соматических заболеваний.

Thus, large industrial enterprises of mining and processing complexes are the main source of pollution in regions with unfavorable environmental conditions. However, as a rule, it does not take into account the fact that a significant part of the disabled and those who do not live to retirement age are former employees of these enterprises, in other words, working conditions are not always considered as a risk factor. Information on the morbidity of workers in the machine-building industry in St. Petersburg has not been published. In this regard, it is interesting to study these indicators and compare them with the incidence in the population as a whole. Objective: To study the influence of working conditions on the level of morbidity among people of working age on the example of workers employed in harmful working conditions of an automobile plant in St. Petersburg. Methods: Within the framework of this work, a retrospective study of three comparison groups was conducted: group 1 (n = 1999 men aged 20-60 years, median age-32 years) - employees of an automobile manufacturing enterprise in St. Petersburg who underwent a periodic medical examination in 2019 at the Eco – Safety MC LLC; group 2 (n = 424,770 men aged 21-60 years, median-38 years) - persons of working age who in 2019 passed a medical examination in medical organizations of St. Petersburg at the place of attachment; group 3 (n = 604,668 men aged 21-99 years, median age 44 years) - the adult population of St. Petersburg, who underwent a medical examination in 2019 similar to group 2. The intergroup data analysis was carried out using the MS Excel 2019 software. The statistical significance of the differences was assessed using Pearson's chi-square test. The differences were considered statistically significant at p < 0.05. Results:In the course of the study, the morbidity of the adult population of St. Petersburg was studied, in the structure of which the following categories prevail in descending order: "Diseases of the circulatory system" (50.1%)," Diseases of the endocrine system "(14.6%)," Diseases of the digestive system "(6.7%) and" Diseases of the respiratory system " (4.0%). The structure of morbidity in people of working age for the prevailing diseases is identical to the morbidity of the adult population of St. Petersburg. The main differences are in the percentages between the groups of diseases: "Diseases of the circulatory system" (41.4%), "Diseases of the endocrine system" (16.9%), "Diseases of the digestive system" (9.8%) and "Diseases of the respiratory system" (4.7%). Nevertheless, in comparison with the group of the working-age population, the trend of higher morbidity of the adult population of St. Petersburg in all classes, except for infectious diseases and diseases of the nervous system, is noteworthy. The incidence of diseases of the circulatory system in the adult population of St. Petersburg is significantly higher. The morbidity rate of automobile plant workers differs significantly from the morbidity rates of working-age people in St. Petersburg, both in structure and in absolute values. In the structure of morbidity of this contingent, diseases of the eye and its appendage predominate, occupying 22.5%; diseases of the musculoskeletal system and connective tissue account for 21.2%; then there are pathologies from the categories " Diseases of the respiratory system "(14.7%) and" Diseases of the circulatory system " (14.4%). When compared by absolute values, employees of the enterprise have significantly higher levels of detection of diseases in all the ICD 10 classes considered in this work, except for diseases of the nervous and genitourinary systems, the incidence of which is significantly lower. The reasons for this difference may be harmful and / or dangerous working conditions at the automobile plant, the peculiarities of the lifestyle of employees, or differences in the conduct of preventive medical procedures in comparison with medical examinations. Conclusions: Special attention should be paid to the health of workers engaged in work with harmful and/or dangerous working conditions. The introduction of corporate health promotion programs has a huge impact on the preservation of the health of employees, which is a powerful mechanism for preventive measures against the development of both occupational and other somatic diseases.
