Determination of phenotypical characteristics of chronic obstructive pulmonary disease in elderly patients

Хроническая обструктивная болезнь легких (ХОБЛ) - одна из главных проблем, затрагивающих здоровье населения по всему миру, и одна из основных причин мировой заболеваемости и смертности с нарастающей распространенностью и нанесением значительного экономического ущерба во всех развитых странах. За последнее десятилетие в пульмонологии чаще стал использоваться термин «фенотип ХОБЛ», отражающий различные клинические типы пациентов с одним и тем же заболеванием. Данное понятие помогает раскрыть патогенез болезни, характер ее течения, прогноз и эффективность методов лечения у разных групп больных. Начиная с 2005 года, выделено несколько фенотипов ХОБЛ, среди которых наиболее распространены: фенотип без частых обострений, с частыми обострениями, с эмфиземой и перекрестный с астмой. Цель данной дипломной работы состоит в определении клинико-функциональных признаков описанных фенотипов ХОБЛ у пожилых пациентов города Санкт-Петербург. В исследование были включены 54 пациента с установленным диагнозом ХОБЛ, наблюдавшихся в городском пульмонологическом центре в период 2017-2018 г. Средний возраст больных составил 67±6 лет. Проведен ретроспективный анализ историй болезней с выкопировкой результатов лабораторных и инструментальных исследований, данных опроса и физикального осмотра. Статистический анализ выполнен с помощью кластерного метода (метод k-средних). На основе отобранных параметров оценки заболевания выделены три группы пациентов. Кластер 1 составил наибольшую долю и характеризуется выраженным нарушением бронхиальной проходимости и диффузионной способности легких при наличии нечастых обострений. Кластер 2 составил наименьшую долю и описывается еще большим снижением функции внешнего дыхания с частыми обострениями. Кластер 3 представляет группу без частых обострений и признаков эмфиземы. Были определены признаки трех фенотипов: с нечастыми обострениями и эмфиземой (кластер 1), с частыми обострениями и эмфиземой (кластер 2) и без частых обострений, без эмфиземы (кластер 3). Таким образом, в ходе работы были проанализированы параметры оценки ХОБЛ и определена их информативность для фенотипической стратификации пожилых пациентов. Наиболее информативными показателями оказались: число обострений в год, ОФВ1, индекс Тиффно, диффузионная способность легких. Определены признаки трех фенотипов ХОБЛ у пациентов пожилого возраста, влияющих на прогноз и выбор оптимальной терапии. При этом самый неблагоприятный прогноз имеют пациенты с частыми обострениями и эмфиземой. Выделение описанных групп позволяет предоставить персонализированное лечение в зависимости не только от степени обструкции дыхательных путей, но и от фенотипа. Это позволит достичь как лучших клинических результатов, так и повысить экономическую эффективность проводимых лечебных мероприятий.

Chronic obstructive pulmonary disease (COPD) represents a major health problem and important public challenge all over the world, including a significant economic damage in all developed countries. In recent years, the term "phenotype" has been used to refer to clinical types of patients with COPD. The term "phenotype" in this field provides prognostic information and allow the determination of appropriate therapy aiming at modifying clinically meaningful outcomes. Among all phenotypes described since 2005, the most widespread are: the non-exacerbator, the acute exacerbator, the emphysema-hyperinflation and the overlap COPD-asthma syndrome. The aim of this study is to define clinical and functional features of well described COPD phenotypes among elderly patients in Saint-Petersburg. There were 54 patients with established COPD diagnosis, who had been observed in the city pulmonary center during 2017-2018 years. The mean age was 67±6 years old. For each patient an in-depth history of the disease was obtained, including laboratory and instrumental data, especially spirometry (forced expiratory volume in 1 s FEV1, Tiffno's index, diffusion capacity DLco/Va). Besides the information about COPD symptoms, smoking status and the number of COPD exacerbations was collected. The statistical analysis was performed with the use of cluster method (k-mean). On the basis of examined variables there were identified three clusters. The cluster 1 represented the major part of patients and characterized by significant airflow obstruction and disturbance of diffusion capacity without frequent exacerbations. The cluster 2 represented the smallest group of patients, who demonstrated the most severe decline of lung function in comparison with other clusters and had frequent exacerbations. The cluster 3 was described by the low number of exacerbations and absence of emphysema. In this way there were identified characteristics of three COPD phenotypes: non-exacerbator with emphysema, exacerbator with hyperinflation and non-exacerbator without emphysema. Thereby, the parameters for evaluating COPD phenotypes were analyzed, as well as their informative value for stratification of elderly patients. The most informative variables were: the number of exacerbations per year, FEV1, Tiffno index and lung diffusion capacity. There were identified characteristics of three COPD phenotypes in elderly patients with prognostic and therapeutic significance. It is important, the phenotype with high exacerbations and hyperinflation has the most unfavorable prognosis and requires close attention of physicians. In this way, we may take on a more personalized treatment according to not only the severity of the airflow obstruction, but also conditioned by the clinical phenotype. It will allow us to achieve not only the best clinical results, but also a favorable economic efficiency.
