Medical care difficulties in rosacea patients

Abstract

В дипломной работе представлены ведущие дефекты оказания медицинской помощи пациентам с розацеа. Розацеа представляет собой хроническое рецидивирующее заболевание с неизвестной этиологией и патогенезом, характеризуется поражением кожи центральной области лица и нередко приводит к образованию выраженных косметических дефектов. Для заболевания не разработаны специфические диагностические методы исследования, и в настоящее время отмечена тенденция к развитию резистентности к общепринятым методам лечения с непродолжительной ремиссией. Перечисленные особенности дерматоза диктуют необходимость иметь представление о типичных дефектах оказания медицинской помощи пациентам. Цель работы: анализ качества медицинской помощи пациентам с розацеа. Материалы и методы исследования: для оценки качества медицинской помощи проведен сбор и анализ необходимых данных из амбулаторных медицинских карт 131 пациента с розацеа. Результаты: при формулировке диагноза форма розацеа была отражена в 90,8% случаев, степень тяжести – в 59,5% случаев. Гинекологический анамнез был собран у 11,7% женщин, консультация гинеколога назначена в 10,6% случаев. В 43,2% пациентам была рекомендована терапия, соответствующая форме и степени тяжести розацеа. Выводы: типичными дефектами формулировки диагноза является отсутствие указаний на степени тяжести (40,5% случаев); ведущий дефект сбора информации - отсутствие назначения женщинам консультации гинеколога (89,4%); основной дефект лечения - назначение терапии, не соответствующей форме и степени тяжести розацеа (56,8%).

Background: The thesis presents the leading defects in the provision of medical care to patients with rosacea. Rosacea is a chronic recurrent disease with unknown etiology and pathogenesis, is characterized by lesions of the central facial skin and often leads to severe cosmetic defects. No specific diagnostic methods have been developed for the disease, and there is now a trend towards resistance to common treatment methods with short remissions. These features of dermatosis dictate the need to be aware of the typical defects in patient care. Objective: To analyze the quality of care for rosacea patients. Research materials and methods: 131 rosacea patients' outpatient medical records have been collected and analyzed, and the quality of care has been assessed on the basis of these data. Results: The form of rosacea was reflected in 90.8% of the cases when the diagnosis was formulated, and the degree of severity was reflected in 59.5% of the cases. A gynecological history was collected from 11.7 per cent of women, and a gynecological consultation was scheduled in 10.6 per cent of cases. In 43.2 per cent of cases, rosacea patients were recommended for treatment in accordance with the form and severity of rosacea. Conclusions: Typical defects in the formulation of the diagnosis are the lack of indication of the degree of severity (40.5% of cases); the leading defect in the collection of information is the lack of appointment of a gynecologist (89.4%) and a gastroenterologist (40.7%) consultation for patients with an aggravated comorbid background; the main defect in the treatment is the prescription of therapy that does not correspond to the form and degree of severity of rosacea (56,8%).
